Leaders for Life – Graduation Cord Program
Versiti’s Leaders for Life – Graduation Cord Program (the “Program”) is sponsored by the Versiti Blood Center of Wisconsin, 638 North 18th Street Milwaukee, WI 53233 (the “Sponsor”). The Program is subject to all federal, state, and local laws and regulations. Void where prohibited or restricted by law.
PURPOSE: The Program is designed to create awareness of the need for a strong and consistent blood supply within our community. By volunteering to support local blood drives, high school seniors will be recognized for their efforts with a Leaders for Life Graduation Cord and a Leaders for Life Certificate.
ELIGIBILITY: The Program is open to current high school seniors residing in the states of Illinois, Indiana, Michigan, Ohio and Wisconsin.
HOW TO PARTICIPATE: To receive a Leaders for Life Recognition Cord and Certificate, eligible students must complete the following three (3) tasks:
- Complete six (6) credits of volunteer service. Service under the Program can take two different forms: donating blood or completing Versiti service hours. Whole blood donation = 1 credit, Double red donation = 2 credits, and a Summer donation (June - Aug) = 2 credits. For service opportunities, see your Versiti Account Representative or Blood Drive Coordinator. 2 service hours = 1 credit.
- Complete on-line Cord Program Application Form. Eligible students who have completed Step 1 above should go on-line to versitileadersforlife.org and locate the application form, complete the application form in its entirety and click “Enroll in Leaders for Life Cord Program.”
SUBMISSION DEADLINE: Eligible students must complete the requirements set forth below, including completing and submitting their application form, by 11:59 p.m. CST on April 15, 2024.
GRADUATION CORDS & CERTIFICATES: Sponsor will evaluate all application forms received from eligible students. For each eligible student who has, in the judgment of the Sponsor, satisfied the tasks outlined above, Sponsor will provide a Graduation Cord and Certificate to the administrative office or equivalent of the high school listed on the student’s Cord Program Application Form. Graduation Cords/Certificates are expected to be delivered to respective high schools on or about May 1, 2024. Sponsor is responsible for delivery of Graduation Cords/Certificates to the respective high schools; high schools retain the sole discretion to deliver the Graduation Cords/Certificates to students at a time and in a manner of their choosing.

ADDITIONAL INFORMATION: Limit one (1) Graduation Cord/Certificate per eligible student. Graduation Cords/Certificates are non-transferable and no cash-outs will be provided. No substitutions of recognition awards unless determined in the sole discretion of the Sponsor. All taxes and all costs and expenses (if any) not specified herein are the sole responsibility of the participants. The Released Parties, defined below, are not responsible for lost, late, misdirected, or blocked delivery of Graduation Cords/Certificates or incorrect information provided on Cord Program Application Form.
By participating in the Program, eligible students (or, if an eligible student is under eighteen (18) years of age as of April 15, 2024, such eligible student’s parent/legal guardian) agree that any dispute with regard to the conduct of this Program, interpretation of these Terms & Conditions, and/or the awarding of recognition awards shall be submitted to Sponsor, whose decision shall be binding and final.
